Agreement

 

This Agreement is entered into this _______ day of ___________________, 2007 between Structural Management™ (SM) of 1462 Erie Blvd.., Schenectady , N.Y. 12305 and

___________________________________ of __________________________________

 

___________________________________ (Dr. Member).

 

Whereas, SM hereby enters into an Agreement with Dr. Member to provide certain services, allow Dr. Member to become a member in The Structural Management™ Network for which Dr. Member agrees to pay SM, as set forth below.

Now, therefore, in consideration of Dr. Members Agreement with SM, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, and as a condition to SM providing the services contemplated under this Agreement, it is agreed as follows:

  1. Upon joining, Dr. Member shall become a student of The Structural Management™ Network (the “Network”) that will focus on marketing, office procedure and collections for those chiropractors wishing to join. The Network will present Drs. with a procedure allowing them full latitude to more fully use their personal diagnostic and therapeutic procedures, however, within a framework that is intended to provide greater results. All applicable and necessary forms, modules and logos will be provided to doctors upon joining the Network.
  2. Dr. Member agrees to pay SM at a rate of $300.00/month for a period of 6 months, for a total of $1,800.00, and upon completion the Dr. Member will become a Certified Member and will automatically enter into an agreement, payable at $99.00/month, by credit card. All payments will be made/charged on the 15 th of each month.
  3. As a member of the Network, Dr. Member will receive:
      4. a. Immediate inclusion to The Structural Management™ Network e mail database .
      b. A weekly teleconference call reviewing further growth and application of SM.
      c. Full time e mail support from Dr. Maggs (DrMaggs@structuralmanagement.com)
      d. The ability to attend any and all of Structural Management™ Seminars around the country at no additional charge, unless CEU credits are needed
      e. All Structural Management™ Practice Modules ($69.95 each) at no charge.
      f. Use and access to relevant articles and news releases to be used in local communities of member Dr.'s.
      g. Pre-designed power point presentations for industry and sports presentations
  1. Any modification or amendment to this Agreement shall be in writing and signed by both parties.
  2. This Agreement shall be governed by and construed in accordance with the laws of the State of New York . This agreement is effective as of the date above.
  3. No part of the Network may be disclosed or used for any purpose other than as provided for herein in conjunction with your membership in the Network without the express written permission of SM.
